EVANSTON, IL – The Erie Family Health Center has available several employment opportunities for its new health center that will be opening October 22, 2012 at the Lorraine H. Morton Civic Center, 2100 Ridge Ave. This is a new health center being opened by Erie Family Health Center. Positions available include: Community Health RN (Full Time); Dental Assistant (Full Time); Dentist (Full Time); Family Practice Physician, Bilingual (Part Time); Health Promoter (Full Time); and Medical Assistant (Full Time).  
The Erie Evanston Health Center is being created through a $650,000 annual federal grant from the U.S. Department of Health and Human Services.  
Erie will expand its comprehensive service delivery model to Evanston and the surrounding suburbs, providing primary medical services, oral health services and behavioral health services. All services will be located under one roof. The Erie Evanston Health Center will serve as a medical home, allowing patients to establish an ongoing relationship with their provider of choice, receive a variety of health care services and receive referrals for services not offered on site.  
The proposed service delivery model is responsive to identified health needs in the community, thereby increasing access to high-quality health services and reducing the resulting health disparities experienced by the community. More on the new health center here>>>.
